What these forms collect

Customer forms collect contact details, general location, appointment timing, document category and logistical notes. Provider forms also collect business, commission, service-area and self-reported credential information. Completion forms collect job references and reported payment amounts. Do not submit ID images, Social Security numbers, medical details, document contents or card information.

How information is used

We use submissions to review requests, assess provider applications, coordinate potential appointments and reconcile referral invoices. Relevant request details may be shared with a proposed independent provider for that request. We do not use this consent for unrelated marketing. Providers receive only the information needed for the requested service.

Storage and service providers

Records are stored in Netlify Forms and can be exported by the owner to an operations workbook. Hosting services process data to operate the site. Square processes payments on its separate website; this site does not collect card details. Payment reports are not automatic verification of a Square transaction.

Retention and requests

Operational and reconciliation records are retained while needed to manage requests, resolve disputes and meet applicable recordkeeping obligations. To request access, correction or deletion, contact the operator at erikperez081621@gmail.com with your submission reference. Do not include sensitive documents. Requests will be reviewed subject to applicable obligations.

Cookies and measurement

This build has no advertising or behavioral analytics integration. The public website has no customer or provider sign-in. Campaign labels may be kept in session storage for this browser tab to identify how a request reached us; contact details and document information are not stored there. Form fields are not sent to an analytics service.
